Use this issue to discuss patterns for asking users for their name.

Start with the GOV.UK pattern for asking users for their names.

Please share NHS examples here, with your design thinking and any user insights.

We are particularly interested to know about any issues arising with different name formats.

The GOV.UK page has some guidance says this:

“Use single or multiple fields depending on your user’s needs. Not everyone’s name fits the first-name, last-name format. Using multiple name fields mean there’s more risk that a person’s name will not fit the format you’ve chosen and that it is entered incorrectly.

A single name field can accommodate the broadest range of name types, but means you cannot reliably extract parts of a name.”

Names need to be separated in NHS services in order to verify ID against PDS. We need both fields to ensure accurate matching.
