The NHS App team have conducted a Discovery into the in-app feedback mechanism from July - November 2023. We are currently in the technical solution implementation phase.
The screenshots below are taken from the current proposed solution. Both screens include a yellow signposting NHS App pattern on all the screens that accompanies a text field for the user to elaborate on their answers. The signposting card warns users not to enter sensitive personal information in feedback submissions. In the content we also included examples of what sensitive personal information might be to help the user:
Here are some of the user reactions to the yellow warning cards for the Sensitive PI we gathered from the user research sessions that confirmed our assumptions:

Several NHS teams have content that warns users not to enter sensitive personal information in feedback or contact forms. Please share examples in this GitHub issue and we'll consider whether we want to propose a pattern for the content style guide.
