What

To perform a PDS look up the following pieces of data need to be collected:

name
date of birth
postcode (as held by registered GP)

The community have highlighted a need for guidance on a best practice approach to:

asking these questions
explaining why we’re asking them
asking these questions in relation to the service context
coping with match failures

Why
