update frontend library to v5.0.0 #191

Description

Update to the latest version of the frontend library (v5.0.0) following https://github.com/nhsuk/nhsuk-frontend/blob/master/CHANGELOG.md#500---26-march-2021

To do

[x] Remove the logic for fixed_coloumn_footer as this variant is now removed from the frontend library
[x] Decide how we handle the Search autocomplete as this is now removed from the frontend library
[x] Add missing div in the Header which @pflynny spotted
[x] Update the testapp to use latest version of Wagtail (2.12.3)
