From the accessibility audit

We recommend populating both legend attributes with ‘Care setting’ and ‘Topic’ and hiding this visually using the visually hidden class class="nhsuk-u-visually-hidden".

This adds:

a visually hidden legend which describes the filter fieldset.
a quick a11y check for opening and closing filters
