Add documentation with guidance for adopting this rule set

In advance of pushing adoption of this rule set we want to ensure it has great documentation explaining how to do so.

Among other things I added sections to the README.md for:

• suggested flow for adopting in existing repos
• guidance about disabling rules for a codebase or specific violation
