Add rules and comments that have been resolved.
PR#7 was closed due to merge conflicts.

Notes

The style of the rules definition in index.js will evolve as rules are added.

Since these are rules we have already agreed upon I'm going to go ahead and merge this change. @mure @dschwartzni @trstamper go ahead and comment here if the rule descriptions / justifications are good or if there are changes to the wording we should make in follow-up PRs.
