Only output directories that actually need to be created #19

as of r45, kettle-cookbook does incremental document generation. However kettle 
cookbook still attempts to create each target directory beneath the output dir.

It's not really a problem, but it would be nicer (and cause less noise in the 
logs) if we'd only create target directories as needed.

This could be implemented the same way as incremental generation using a merge 
diff step.
