Reorder and label the fields as follows:

Category, Subcategory, Sub-sub-Category, New/Used, Condition, Quantity, Start Date, End Date, Location, Additional Info

Notes: Location: Where do you foresee yourself storing these items? 'I will be holding these items at my home address’ Display Address 1, Address 2, Village/Town/City, County and Country fields. Have a checkbox preselected saying 'I will be storing this item at my home address', with the option to unselect and defaults disappear and fields are editable.
