fix impl ToHeader for MimeContentTypeHeader #25

Prior code was emitting like this, with missing semicolon before the first parameter, and excess semicolon at the end:

Content-Type: multipart/alternate boundary=ywhVrPSWsKaqHhGyVjakP2KZ3xYkmF;
