Add Sphinx theme, appveyor build fix and update files

Overview

• Tell AppVeyor and Travis to completely rebuild docs
• Add theme to documentation
• Add high res logo and favicon to documentation
• Remove files that would break the theme
• Add AppVeyor badge
• Rename Travis and AppVeyor badge to Linux Build and Windows Build respectively
• Tell AppVeyor to use pytest 3.2.5 because any higher doesn't support python 3.3
• Capatilize coverage badge
• Update README and CONTRIBUTE

AppVeyor now also builds documentation for testing

Additional Information

I originally had code in here that removed spaces from class names to stop sphinx from complaining but I removed that.

We may want to change travis pytest installation later as it may break like the appveyor
