Reference marital status #123

removed marital status VS and concept map from VRDR and referenced VRCL marital status instead
added death date example 4 that was missing
updates to data dictionary - first pass, fixed some broken links. will continue to update
used script to generate intro page content

Note: For consistency, I used https://hl7.org/fhir/us/vr-common-library/2024Jan/ValueSet-ValueSet-marital-status-vr.html as the link for MaritalStatus VS in the data dictionary, but it's not working. Should I link to the CI-build instead? I'm imagining the link will change anyway throughout this process..
