Need hazards analysis documents.

Will co-ordinate with dev team, should be mostly tabular list of potential harms, severity, and any mitigations/remedies.

This is one of the supporting docs for https://github.com/nightscout/process-controls/issues/2, there are variety of proposals to do this on per-project basis in the wiki/repo.

@nightscout/owners , @nightscout/dev might be interesting to create simple markdown file with kind of definition list of hazards and remedies. Doesn't need to exhaustive per se, but might be nice to document some of the things we've already done.
