Just a few minor cosmetic requests on the input form

Remove deprecated terms from input form

There are a few outdated options remaining in the input form. Here is a list of the variables and their options which should be removed:

Sector Neglected Tropical Diseases (Nt Ds) It looks likes some data may have been uploaded with a typo in the sector, including a space in NTDs: "Neglected Tropical Diseases (Nt Ds)". I think don't the original Excel spreadsheets had that space.

Activities: Vitamin A Distribution

Disease: Soil Transmitted Helminthiasis Please keep the version with the hyphen after soil

Audience: Pre-school Children Please also change "preschool age children" to have a hyphen between preschool and age.

Info Bubbles on Input Form

Most variables on the input form have an "info bubble." Currently, the text inside many bubbles extends beyond the edge of the window itself, making it unreadable. See the "Description" variable as an example.

Additionally, we can include text for info bubbles on the new variables we added:

Activities: What activities are being carried out by this project? Please choose all options that apply.

Audience: Who is the activity aiming to reach? Who is the audience or target population? As a general guideline, infants are considered to be aged zero to one year, pre-school children (PSAC) are one to four years, and school-age children (SAC) are five to fourteen years. These target groups do not need to perfectly align with your activities' target groups, so please use your best judgment.

Diseases: What disease(s) do your efforts target or involve? Please check all that apply.

Medicine: What medicine, if any, is involved with your project? Please check all that apply.

Mandatory fields

Can we make contact name and contact e-mail mandatory input fields? Alternatively, let's at least put an asterisk next to their variable headers so that people think they're mandatory fields.

Looks like the NTDs sector is still showing up twice in the input form. Any ideas?
