Installation Failed

What steps will reproduce the problem?
1. Copied Genus folder to local webserver folder (wwwroot)
2. Opened the URL in the browser and successfully passed the Step 1
3. Populated and submitting the second form (database configurations)

What is the expected output? What do you see instead?
Instead of creating the database, I encountered following error:
PHP Notice:  Undefined index:  create_database in
C:\Inetpub\wwwroot\genus\install1\step2.php on line 38
PHP Fatal error:  Class 'mysql' not found in
C:\Inetpub\wwwroot\genus\install1\step2.php on line 45

What version of the product are you using? On what operating system?
genus-2.0.0beta on php-mysql enabled IIS on Windows XP

Please provide any additional information below.
There is no help documentation to troubleshootthis issue
