Added 'forms_builder.forms' to installed_apps req #4

Corrected the INSTALLED_APPS requirement for the app setup.

Nice catch. cmsplugin_forms_builder doesn't even need to be in INSTALLED_APPS at all, just forms_builder.forms. I'll merge this and remove it. Thanks for the contribution.
