Path planner is allowing dwarves to path out #189

The path planner is allowing dwarves to path out of "stuck" regions. 
Essentially if a dwarf is stuck in a hole, sometimes the planner allows 
movement outside. This breaks the run-time movement logic since it moves 
through blocks, etc..

Trivial mistake: I forgot to take care of the simple case of "what if the 
source is the sink". ... *For shame Jeremy, you should know better :P*
