ompt_get_task_id/Leightweight teams failing in nested regions #4

If you have a region with 1 thread and a nested regions with more than 1 
thread, the ompt_get_task_id function will find the lw_team structure and 
return wrong task ids in the nested region.

The testcase test_task-id-simple shows that (as soon as I manage to push my 
update to it)
