CSS Classes - PopUp - SS2.4.2 #2

What steps will reproduce the problem?
1. Installation of SS2.4.2
2. Testimonial Example
3. Add, Edit and Delete action

The form was not opened in a PopUp - this is referrable to wrong CSS classes.
(see attached patch file)
