Refactor: Reduce PR templates to only one #115

Overview

This PR narrows down the multiple PR templates to one, the previous default.md and places it directly in the .github/ directory.

Changes

Removed the merge.md
Took the default.md out to the PULL_REQUEST_TEMPLATE/ directory and placed it directly in the .github/ directory
Rename the default.md to pull_request_template.md to be applied on a new PR creation
Fixed typo and wording in the template

Review points

We have to merge this PR first to confirm whether this PR template is applied when a new PR is created.
Thus, please merge it unless you find a fatal issue.
