Reinstate outputs for census-merged LADs #20

so that City of London and Scilly Isles are in outputs. (Just adjust Westminster/Cornwall proportionately)

Synthesise migrations to/from CM LADs by proportionately adjusting migrations according to Scilly/Cornwall and City/Westminster population ratios, then round and convert to int (model obs must be integer)
