Improve the location information for transmitters #29

Mark each transmitter as being in
- England
- Northern Ireland
- Scotland
- Wales
Add Ceremonial/Lieutenancy counties

Possible sources of data:

Added list of transmitters for each County: ecd94f09c8cb6470616fba8f6a996bba9bee6be0
