Feature/add utrecht bem classes

This pull requests contains the following changes:

the .env file in the repository has been removed and added to gitignore. Instead, an .env.example has been added so developers can have their local .env file without conflicting commits
FormIO templates and components have been modified to use Utrecht BEM classes instead of open forms BEM classes. The Utrecht BEM classes add support for design tokens
The NLDS Theme Switcher has been added. Running yarn start will now serve up an application with the theme switcher at the top.
For several components, such as the React buttons which reside outside of FormIO, CSS and JSX changes have been made to support design token theming.

With the exception of the Button react component, all non-FormIO components will be handled in a separate PR to keep the changes oversee-able and implement design tokens in iterations.
