Module Creation: File uploads #181

When i tried to create a new video module its not moving me to the course page and i get this server output. I did attempt to upload a .jpg and .gif before the .mp4 so i dont know if it has something to do with that:

This seems to be happening for anything file upload related, this is for PDFs. I did the same thing as the video, tried two different file types and then a PDF:
