NMRS contact and Care Tracking Form to include Tx_ML outcomes

Hello Colleagues, I think we should take care of this right away.

NMRS Contact and Care Tracking Form needs to include the following Tx_ML outcomes.

• Undocumented/self transferred out
• Tracked unable to locate
• Have not attempted to trace yet
• Traced but refuse treatment
• Traced and returned to treatment

"Issue" The provision for "other" does not currently allow typing of the detailed reason as require for Tx_ML reporting.
