It may not be posible to code all the signs and symptoms, regimens, lab tests, drugs. These concepts should be accompanied by an empty text field to capture their NON CODED valued that the Doctor might want to type.

@brightoibe In as much as this might be a good way. lets also think of analyzing the results. if we create free texts, we might end up having some unusable data. Meanwhile, My take again on this will be to follow the National tools and only make slight changes when necessary.
