Remove "Visit Date" Default Date as "Today's Date"

Visit Date on most of the PMM forms usually appear with the current date by default. This was done with the assumption that the "Data Entry Date" and the "Encounter Date" of PMM Forms will be the same date. But based on observed human behavior , DA's will submit the forms like that without changing. I propose that the deafult=now attribute in the tag;

be removed to reduce human error. This can affect all Viral Load coverage as DA's will submit a Lab Form with the Date of Data Entry instead of backdating this date to the Date of Specimen Collection.
