updates on client Tracking and Discontinuation Form: #344

  1. Created a dedicated section for the Client Verification.

  2. Modified and increased the numbers of "Indication for Client Verification (triggers)" to 10 with the option to specify any other option.

  3. Created a dedicated sub-section for "Verification Attempts and Steps" which will capture options for "Date of Client tracking", "Verification Attempts (you can select multiple triggers)", "Verification Status", "Outcome", "Comments (Provide reason for records discontinuation)".

**Included the JSON updates for "Client Tracking and Discontinuation Form" and "Care card"
