Add the ability to disable Mac Mouse Fix on a per-application basis [enhancement] #598

Description

A new preference pane with a table view allowing the user to add specific installed applications to a blacklist. While the ability to add per-application mappings would be great, this is more complex and is detailed in #588.

Rationale

Many 3D modelling applications, e.g. Autodesk Fusion 360, Blender, etc., require the use of the middle mouse button to control the view camera. Mac Mouse Fix breaks this when, for example, clicking and dragging with the middle mouse button is remapped to switching Spaces.

This would also be very useful for me for things like game streaming. I use Moonlight to stream games from my PC, and many of those games use the middle mouse click or other mouse buttons for specific functionality. I'd like to disable Mac Mouse Fix entirely when I open Moonlight, and re-enable it automatically when I quit or switch apps.
