Custom Field 'Create Subscription Form and get Code' function is not working #279

I'm using WP v3.5. In Wp-Autoresponder I can create custom fields OK. Once I go into Create Subscription form and tick the fields I want to add to my form and click on 'Create Form and Get Code,' I get no change to the code. It just shows the default Name and Email. Also Preview shows the same. When I go to Subscribers everyone of the Custom form fields is showing up under a test subscriber with no data. I have tried the form creation on three browsers with the same results. What needs to be done to get the code so I can copy to my Web page?

All started working after a number of refreshes and restating of Wordpress a number of times. I can now get the HTML code and edit it for a complete customized Subscription form.I tried in a number of different browsers but did not work. Now working happily in Firefox.
