Just support current md file！ #116

If open multiple markdown files, just one md file can use this plugin, the other will not be able to.

Actually only the first MD file that invokes it will be able to use it.

The only way I got it to work with multiple files was to close atom and reopen it, but that was to create a TOC in an empty file. To update an existing TOC, I needed to close and reopen. Call markdown toc: toggle, then undo insertion. For some reason the automatic update didn't seem to work if markdown toc: toggle wasn't called.
