Thanks for letting me know. Could you please provide the following:
I tried this under Greebo with the a freshly installed Mikio and indexmenu using the basic syntax {{indexmenu>.}}
and got a totally different result with no icons and completely different font face/colours, so I need to know a bit more detail to see what's going wrong

The version of Dokuwiki you are using? I don't have access to the wiki right now. I can give you the exact version later today. I set up the wiki 2 weeks ago so it's a rather recent one.
Which indexmenu plugin you are using (there are several)? I'm using the following plugin: https://www.dokuwiki.org/plugin:indexmenu
Have you made any changes to the template style settings or the mikio style sheets/templates? No.
The syntax you are using in your sidebar?
I forgot to mention, i'm using the javascript (js) option, since normally they work a lot better in my opinion. I believe the non-javascript (default) ones worked fine. So the syntax is something like {{indexmenu>#1|js#icons8metrobw.png}}

Cool, the syntax {{indexmenu>#1|js#icons8metrobw.png}}
shows the same formatting error for me as well. I will take a look at it and see what I can do over the weekend
I'm using Mikio in combination with the indexmenu plugin. The indexmenus work fine, when they are on the page itself. However, when they are in the sidebar, they are completely broken (see screenshots). https://imgur.com/a/M1Ryzv3 The problem doesn't occur when using the default dokuwiki theme or the bootstrap3 theme.
