Enhance Integrations table #43

@larsonel suggested some improvements to the integration table:

  1. Date of creation
  2. Clinic ID (Antech)
  3. Last Start/Restart date/Time?
  4. Ability to "copy" the integration ID and then paste it into the Event Filter for Integrations

@monsieurBelbo Would it be useful if we were able to search (either a textbox or a dropdown) by hospital? For instance, searching for all the integrations in "1326 VCA Bay Park Animal Hospital".
