Mdebug #2419

Get MDEBUG working and producing sensible output. There's more work to encapsulate debug allocations with size/tag (instead of two additional malloc calls - and linear list search) to come, figured get this merged so following changes are more easily reviewed.

I would pull the contents of plibrary into loadkeylib so that the assumptions about BUFSIZ is all in one place.
