Fix choosing RTP sequence number and synchronization source #57

Ensure that RTP synchronization source id and initial sequence number is unique per RTP stream.

Thanks for this and all your other changes and sorry for the very late response. I've made some small code style changes in 6631752 but otherwise the change is takes as you provided it.
