fetch: add request/response info for debugging #122

This is ported over from https://github.com/npm/npm/commit/444d0c1aa55e04fd9d959de7f22f43a0a4ae7b86 to prevent the manual hack.

I'm not sure it's worth writing a test for something like this that's meant more for convenience-debugging, but I'll leave that to the reviewer to call. It's still nice to have this.
