Steps to reproduce

  1. Visit admin page
  2. Add a maintainer

Expected behavior

Success message, form cleared

I think it should anyway, not clear if this is a general bug with forms, or perhaps this specific form decides it should remember the last submission.

Actual behavior

Success message, form remains populated
