Update LaTeX TR #39

Other things. Typos. This ticket is for tracking this big effort.

I pulled these comments from the cls file:

% The easiest way to include a .sty file is to place it in the same folder
% as this .cls file.  However, if you include it in your LaTeX installation
% folder, a manual installation step must be performed:
% Important note: when you install a .sty file into your LaTeX
% distribution, you need to update the FNDB for LaTeX to
% recognize the new files exist.
% MiKTeX users do:
%   Run Start - Programs - MiKTeX 2.9 - Maintainance (Admin) - Settings
%   (Admin)
%   Then click the "Refresh FNDB" button
% Other distributions (MacTeX, TeX Live, etc)
%   Try mktexlsr or texhash on the command prompt

Seems like it might be nice fodder for a section in the TR, if its not already there.
