data: recode various missing observations; better organisation #39

Move code that creates df_complete, the filtered and selected data frame used for analysis from index.qmd to r/shf_thy_nod.R.

Copies code added to index.qmd on branch trial-imputation that recodes NA to No into r/shf_thy_nod.R so that all data cleaning and recoding is done in one place.
