Closes #804.

add PFP event EV_EFI_NOACTION_NvIndexInstance which has an SPDM component in it
add SPDM Cert Chain data structure
the test pattern for the NvIndexInstance event includes a test pattern for the Cert Chain data structure so both can be tested with this
cleaned up output for several other events
updated error checking to several events
