SHHS MOP 6-AB-mop-sample-qs-form Appendix B – Sample of QS Form, navigation takes you to this page with button “Sample QS Form”, when button is clicked, navigate to Forms page with two more buttons “shhs1” and “shhs2”, there is a list of forms for each but neither list has an example of the SHHS QS form. SHHS1 list of forms does have PSG scoring notes form, but this is like page 2 of the QS form. Page one should have the QA grades given to each of the signals and overall QA grade for study.
I did send a PDF of SHHS 1 QS form [2 pages] that Susan Surovec sent me to Mike and Remo. I put copy of the PDF at bottom of this issue.
