updated sl3 version to handle competing risks #118

Added a new parameter comp_risk to lmtp_* estimators. This is a longitudinal indicator variable similar to outcome in time to event settings. The indicator affects the at-risk subset of data for estimation.

A lot of changes have been made to the internal structure of the package and the sl3 branch is super behind. I've gone ahead and made the change in this pull request to a new branch that's caught up with devel. Do you have any data available for testing and adding an example?
