Problem seems to occur about 1 in 4 times. The firmware upgrade can be recovered by manually removing and reinserting the drive or restarting the upgrade at another time but firmware upgrade needs to be the most reliable process in the firmware. Fixing issue #46 might be a way to resolving this issue since it might be the case that initial USB handshaking will never be completely reliable.
