Fault Tolerance Refinements

• [X] Don't requeue txs after failure since order can be of significance. Txs remain in their position in the queue in spite of failure, unless the user takes more drastic action
• [X] on_broadcast_failure and on_fault callbacks are now mandatory when queueing a transaction. It ensures that the user has thought about what to do during a failure scenario.
• [X] A reverted tx (receipt with status 0) is not actually a fault, it is really just a FinalizedTx that was unsuccessful; don't treat it as a fault anymore. As a result the on_finalized callback is now mandatory when queueing a transaction since the user should handle the case where the finalized tx was reverted / unsuccessful.
• [X] Be more efficient with RPC calls as they relate to checking transaction confirmations.
• [X] Allow the speedup strategy to specify the minimum time between speedup increases - allows for more control over overspending for a tx.
• [X] Provide separate mandatory callback for the case of insufficient funds - fixes #13.
• [x] Update repos diagram to reflect updates.
