Adds support of ehrbase/fhir-bridge as CRR GECCO FHIR Endpoint. Fixes Issue with MII lab-value profile. Improves logging and debug options. Adds option to enable client verbose logging while accessing the fTTP and GECCO FHIR server. Adds options to configure timeouts.

Adds blaze and fhir-bridge to the integration test setup.

Closes #22
