Revise Documentation #34

Adds documentation for different evaluation routes. This includes CQL and Structured Query evaluation.

Also adds documentation on supported query types and their corresponding MimeTypes when being sent as part of a Library resource (FHIR).

Adds documentation on obfuscation behavior.

Resolves #22.
