Change the displayed names of questions in sequential mode #852

I've added this, but this problem has immediately appeared: how should we number questions without custom names?

I think that you'd want to exempt a question with a custom name from the numbering, for cases where you want a question called something like "Formula sheet". Are there any cases where it should affect the numbering?

I'm going to close this for now, and we can open another issue if it turns out that questions with custom names do need to affect the numbering.
