Move out from google forms #4

Investigate other options that do not involve google forms.

Can we use NumFocus surveymonkey infrastructure?

Can we spin up something on a LAMP stack instead? I'd like to stay away from proprietary services if possible, especially since we'll be connecting the two forms via a form ID and having varying levels of anonymity.

LAMP wouldn't be my first choice but in terms of Platform as a Service I'd recommend Heroku because their developer experience is well crafted.

Not familiar with the NumFocus surveymonkey infrastructure, can you expand?
