Add (main) governance doc #4

This is the main document under the governance repository, which points (or will point) to other docs. It incorporates what was reviewed in the roles document.

Right. I felt that roles and projects would get a lot of discussion, so I put them out for separate review. This is the content I intend to put in production. It incorporates the previous roles documents but links or will link to Projects, Contributions, Decisions and maybe other things.

Suggested changes to the breakdown in docs are welcome along with any other ideas!

@rprouse I held back from using Organization Leader because I have some thoughts that we may end up with more than one GitHub organization. I could be wrong. I'm merging this, but we can make some final changes when all the files are complete if anyone wants to.
