Thank you for your feedback. We agree that more information such as tags/remarks could possibly be useful for doctors. However, one of the issues identified from our research about EMRs (Electronic Medical Records) for doctors is that there are too many compulsory/mandatory fields to fill in that is actually taking up a lot of their time.
In particular, one of our research sources mention this issue explicitly about not being able to "skip data entry fields":
"Technology can take up too much time from physicians. A recent research letter in JAMA Internal Medicine found attending physicians reported losing an average of 48 minutes of free time per clinic day due to EMRs. With already busy schedules, maximizing available time is critical for physicians. Dr. Pearl wrote many physicians also become frustrated with software that doesn't allow them to skip data entry fields. He suggested entrepreneurs can focus on developing software applications with macros and smart lists to help reduce the time physicians are entering data into the EMR." Hence, the decision to make fields that are not absolutely necessary was based off this concern to make our application more user-friendly for doctors.

For private clinical doctors, it is often more useful to have compulsory tags or remarks in order to make the information about appointments more useful, so that it's not just an AddressBook with extra information.
